Create your first form

Bodek Forms lets you build forms and surveys and collect the answers in one place. You'll need a Bodek plan that includes Forms.

Start a form

1. Sign in at forms.bodek.us. You'll land on My Forms.
2. Click New form.
3. Replace Untitled form with your form's title.

Add questions

Each question is a card with the question text and one or more answer fields:

1. Type your question where it says Type your question here….
2. Pick an answer type from Answer types in the side panel, such as Short text or Multiple choice. Tip: click a question first, then an answer type, to add it to that question.
3. Click Add a question for the next one.

Drag the grip handle on a card to reorder questions. Use a question's ⋯ menu to Duplicate or Delete question.

To add headings or instructions between questions, use Title, Subtitle or Paragraph / list under Content.

Save and preview

Click Create form the first time. After that, your changes save automatically; you'll see Saving… and then Saved. Click Preview to open the live form.
